Requesting Your Health Care Records
You have the right to request a copy of your health care records. Please submit a written request to your therapist. Please include:
- Your full name
- Phone number
- Date of birth
- Current email address
- Address to which you wish the records to be sent
Please also state the specific records you would like to receive.
